What does Clara's escape foreshadow in the novel, What She Left Behind?

Clara, with the help of Lawrence, hides in a coffin to attempt escape from the asylum. While Clara does not die in the attempt, the coffin nevertheless acts as a symbolic harbinger of bad things to come, including the death of Lawrence and Bruno during the break-out.
