What metaphors are used in What Jamie Saw by Carolyn Coman?

The question of whether or not Van really threw the baby across the room with Patty catching her remains unanswered..... did he literally throw the baby across the room, or is this really a metaphor for how Patty realized she had to save Nin and Jamie and herself from the abuse?
