What event led to Nana's addiction to opioids in the novel, Transcendent Kingdom?

When Nana was a sophomore in high school, he tore ligaments in his ankle during a basketball game and was prescribed opioid pain medication by an emergency room doctor. He became addicted and when he could no longer get a prescription for this medication, he started doing street drugs, including heroin.
