How does Doctor Heine's coat catch fire in the novel, The White Horses of Vienna?

The innamed doctor's wife advises Dr. Heine to stand behind the patient, as her always does. He acquiesces, but as he moves position, his coat catches the flame of the sterilizing lamp.
