How is Chillingworth able to see what Dimmesdale is hiding on his body?

Chillingworth has to wait until Dimmesdale falls asleep before he creeps up on him and examines his chest. It is only after he has spent a long time lulling the minister into comfort with medical treatment and advice and then to sleep that he is able to creep up and look under his shirt.
