Sacks first observes the patient when he is admitted, finding him in a reasonable state. He then hears from the nurses that something is wrong and he finds the patient lying on the floor, angry and agitated. The main character has decided that his leg is not his own and wishes to remove it. Apparently someone has replaced his leg with someone else's, at least to his mind.
