How did Mike die in the novel, The Killer Inside Me?

After he was released from prison, Lou's father arranged for Mike to have a job at a construction site. It was while working at the site that Mike slipped from some scaffolding and died. However, many people believed he was pushed and Lou thinks the man who arranged it was Chester Conway.
