What is the setting of Survivor by Chuck Palahniuk?

The book begins with Tender on Flight 2039, which is bound for Australia. There are leftover food trays and personal belongings scattered everywhere. Throughout a couple of the chapters, Tender interjects on his own story, and the reader is brought back to the status of the plane.
