What does the carved heart that Atlas gives Lily represent in the novel, It Ends With Us?

The carved heart is a symbol of love and thankfulness. Atlas gives Lily this heart as a gift to thank her for taking care of him. Lily fears it is a gift to remember him by because Atlas has told her that he will be leaving to stay with his uncle in Boston.
