During his fourth year of medical school, and four months after becoming married, Nicholas goes to Winslow, Arizona to work at a clinic on an Indian reservation. Nicholas is overwhelmed with the poverty and lack of medical supplies at the clinic. On his final day there, Nicholas treats a young girl who is pregnant; the baby is in a breach position. Nicholas wants to perform a C-section on the girl, but a Hand Trembler comes in and runs her hands over the girl's belly. The baby is born in the normal position.
