What was the reason the two dragons couldn't come apart?

The reason the two dragons couldn't come apart was two fold. The first of these was that Khirsah had the blood lust upon him and couldn't tell what was going on, other than he had to kill his opponent. The second of these reasons was that the dragonlance had stuck in the blue dragons shoulder and lodged there tight.
