Why does Dr. Hoskins feel so guilty in Part Seven?

As someone who does not believe in slavery, Dr. Hoskins feels that remaining silent about Noah and the baby's murders makes him just as guilty as Mr. Derby himself. The doctor knows he cannot do much to end the whole institution of slavery, but he wishes to make an impact now by helping the children with a chance of escape.
