1. What constitutes Wesley Dodd's final moments?

Norman McKay reads from Revelation to Wesley. As he rips out his IV, Wesley begins to tell his pastor that he has visions of the end of the world which is coming soon. However, his speech is cut short by the doctor and nurse who are trying to fix his IV. Knowing that he is close to the end, Wesley passes on his powers as the Sandman to the nurse and doctor, ensuring that they will always be able to help their patients rest easily, as well as passing on his visions to Norman.

2. What leads up to Wesley Dodd asking random people on the street if "they missed the concept of human achievement?" and what did he mean?

Over the years, as the superhuman population grew, being human and getting any sort of achievement began to be considered mundane when compared with people who could fly. In the artwork, one can see Norman McKay walking past an advertisement for the baseball used in the last World Series, indicating that competitive sports has ended. Wesley's question shows that he understands that the humans and their pride have, effectively, been wiped away since the superhumans took over.
