What do the altars in the novel represent? How do they relate to people in real life?

How difficult are the various parts of the journey compared to one another? An example being, the beginning of the story where Much-Afraid made the decision to follow the Shepherd vs. the final part of the journey where she must give over her need for human love as a burnt offering. Do the challenges she faces get progressively more difficult? Or, is the difficulty collective, in that as she tires from the journey the challenges seem more difficult? In any case, what is it that gives her the strength to continue?
